About the Opportunity

Key Performance:

Responsibilities

  • Facilitation and support of the reporting and investigation of Adverse Events Following Immunisation (AEFI) reporting and investigation in the province and district.
  • Alerts the National Department of Health, Expanded Programme on Immunisation AEFI Coordinator of the serious/severe AEFIs.
  • Assign EPID numbers for reported AEFI cases in consultation with the district public health officials.
  • Assist with the follow-up of cases and ensure all Case Report Forms (CRF), Case Investigation Forms (CIF), and supporting documentation (clinical notes, laboratory results, autopsy, verbal, post-mortem, etc.), and line lists are submitted to National AEFI Coordinator, and captured on the electronic system (MedSafety-Vigilance Hub).
  • Monitor the timely submission of CRF and case investigation forms (CIF), and initiate investigation of severe or serious cases, clusters and adverse events of special interest (AESI).
  • The provincial vaccine safety surveillance team follows up and ensure all CRF, CIF, documentation (clinical notes, laboratory results, autopsy, verbal, post-mortem, etc.) and line lists are submitted to National EPI / AEFI Coordinator and captured on the electronic system.
  • Serves as a Vigilance Hub super-user, responsible for drawing of AEFI reports, line lists and ensuring cases are assigned to the district public health officials for follow-up.
  • Train AEFI Coordinators (on Med Safety App and the Vigilance Hub) access and capture AEFIs, reporting forms, add missing information, upload supporting documents, draw line lists for follow-up, and generate reports.
  • Capture paper based CRF on the Vigilance Hub, update the case on the Vigilance hub as investigation is conducted.
  • Upload all paper-based investigation forms, clinical records and autopsy results on the Vigilance hub.
  • Prepares formal communication and AEFI surveillance updates / reports.
  • Assist the provincial team to analyze the CRFs and guide on investigation.
  • Serves as secretariate for the Provincial review and investigation team meetings.
  • Attends National and Provincial AEFI Team meetings.
  • In the event of death occurring within 30 days of investigation alert provincial AEFI review committee and the National Department of Health, for further guidance.
  • Submit CRF, CIF and clinical records and if applicable submit Med App Safety reference number.
  • In the case of death occurring within 30 days after vaccination ensure necessary post mortem examinations completed.
